File an Insurance Claim with United American Insurance Company

Filing a claim with United American is simple and easy, because we truly want to help you when you need us the most. To file a Medicare Supplement, Supplemental Health, or Life Insurance claim with United American, see the table below for the required forms and address for submitting documentation. Please submit all necessary forms together. Insurance claim processing can be delayed while waiting for additional paperwork if the required forms aren’t sent at the same time.

Available Medicare Supplement Claims Filing

Electronic claim submission and paper claim submission are available and accepted by United American Insurance Company. United American encourages providers to submit claims electronically to Medicare for claims-based crossover using United American’s Coordination of Benefits (COBA) number 55004.

Universal Billing (UB-04) with copy of Medicare Remittance Advice

All Other Providers

CMS-1500 with copy of Medicare Explanation of Benefits

Certified Death Certificate
Obituary
Name, address, and telephone number of the beneficiary
Copy of police report/coroner's report and newspaper clippings if death was the result of an accident or homicide
Complete the printable Proof of Death Claimant Statement.
