Collection of Information
We collect and retain the following categories of personal information, which is necessary information for us to comply with regulations governing the insurance industry and the issuance of insurance policies. We also collect personal information from our California-based employees, as required to comply with federal, state and local laws governing employers, and to fulfil our legal obligations as an employer. This personal information may include any of the following categories of personal information: (1) your real name, postal address, telephone number, unique personal identifier, email address, social security number, and/or similar identifiers; (2) commercial information, including insurance products or services purchased; (3) professional or employment-related information; and (4) educational information from our employees. We may collect such categories of personal information about you from: your completion of an application for insurance or other forms; your transactional history and other experience with us and other companies; and in the case of an employee, from an application for employment, curriculum vitae or bio, or from a consumer reporting agency. All such personal information is used to comply with federal, state and local laws, and regulations governing our insurance business, and for the business purpose of complying with laws governing the employment of persons who are our employees, and to effectively recruit and manage our company work force.
Access to, Correction of, or Requesting Deletion of Personal Information
If you are a California resident, upon our receipt of a verifiable request from you, you can obtain access to the personal information about you which we have in our files, or which we have used or shared with third parties. If you think that any piece of personal information that we have collected from or about you is inaccurate, you may notify us in writing to request that it be corrected. We will investigate your request and make appropriate changes as needed. You may also request deletion of your personal information that is collected and maintained by us, as provided by law.
To submit a verifiable consumer request to know what personal information we may have collected or shared, or to request deletion of personal information, you may call our toll-free number at 1-800-331-2512 or use our online request form or portal at: www2.unitedamerican.com/contact For existing policy or account holders, you may be asked to verify your identify through the existing authentication procedures of your account, and we may re-authenticate you to verify your identity.
For non-policy holders, we will verify a consumer’s identity by matching at least two or three consumer-provided data points with data points that we maintain, depending on the sensitivity of the information involved.
Disclosure or Sale of Personal Information
During the past 12 months, we have shared the contact information of certain consumers with our affiliated insurance companies and with third parties with whom we have joint marketing agreements for the purpose of offering complementary insurance and financial services to our own insurance products and services. We do not sell the personal information of minors under 16 years of age without affirmative authorization.
Your Right to Opt Out
As a California resident, you have the right to limit the personal information that we share with or sell to our affiliates or third parties. You may also limit our affiliates or third parties from marketing their products or services to you based on your personal information that we collect and may have shared with them. This information includes all of the information described above. This is known as “Opting Out.” Your choice to opt out of such disclosures will remain in effect until you tell us otherwise. If you wish to submit a request to opt-out, you can do so by completing the web form located at www2.unitedamerican.com/contact or by calling our customer service department at 1-800-331-2512. If you use an authorized agent to exercise your opt-out right, we will require you to sufficiently identify the authorized agent so that we can verify that person’s identity. (See “Authorized Agent” subsection below). Even if you opt out of the disclosures described in this section, we may still disclose information to affiliates and nonaffiliated third parties as permitted by law.
Right to Non-Discrimination for the Exercise of a Consumer’s Privacy Rights
You have a right not to receive discriminatory treatment from us for the exercise of any of your privacy rights conferred by The California Consumer Privacy Act (“CCPA”).
Authorized Agent
As a California consumer, you can designate an authorized agent to make a request under the CCPA on your behalf. To do so, please write to us at the address provided below, providing your full name and address, the name and address of your authorized agent, the agent’s contact information, specifying your relationship with the authorized agent, and the purpose for which you are designating the authorized agent. You may revoke the authority of that agent at any time by contacting us in writing at the address below.
